In order to make your experience with Dr. Mantas a comfortable and productive one the following is a step-by-step guide to your first office visit.
Step 1

Step 2
Request an appointment with Dr. Mantas using the link provided below. We value your time and make every effort to complete your evaluation and treatment in as few visits as possible. In order for us to be efficient with your time please be prompt to your appointment and always give 48/72 hours notice if you are unable to keep an office/procedure appointment so that other patients on our waiting list may be served. A modest fee will be assessed for those who miss appointments or procedures after this time.
Step 3
Dr. Mantas and his highly trained staff are dedicated to providing you with the best possible care. Please bring the following to your appointment:
- COMPLETED New Patient Registration Packet (if you did not use the Patient Portal)
- Health insurance card and all pertinent information
- Your Photo ID
- Completed registration packet
- A friend or family member, if needed, to take help take notes and ask questions
- An ACCURATE list of all CURRENT medications you are taking including dose
- List of questions concerning your condition
- Copies of records from your doctors including physician notes, tests results, lab work and imaging reports
- Personal calendar to schedule future office visits and procedure appointments
Step 4
Arrive at your appointment 30 minutes early and check in with Dr. Mantas’ Receptionist at the front desk. After a short wait Dr. Mantas’ Medical Assistant will bring you into the patient examination room where she will check your vital signs and obtain a short history. Dr. Mantas will then meet with you to discuss your condition and perform a physical examination.
Step 5
At the end of your visit Dr. Mantas will introduce to his Scheduler where she will assist you in scheduling blood work, endoscopy/colonoscopy, imaging and future appointments. She is also able to answer any questions that you may have concerning procedure preparation and insurance issues. Please allow 1-2 hours for your appointment.

After your visit please revisit this website to learn more about your disorder and procedures scheduled with Dr. Mantas.
